Chiropractic Care Is Affordable!
At David City Chiropractic, we accept various types of payment and insurance from many sources to make chiropractic care affordable for any budget!
Our staff is well trained and experienced in all types of insurance coverage for chiropractic. Your insurance will be filed by the fastest most efficient method available, either by electronic submission or computerized forms.
We treat (accept) patients with any of the following types of coverage:
- Individual or Group Insurance Plans
- Managed Care Plans (PPO’s & HMO’s)
- Medicare (Part B)
- Medicaid (Nebraska)
- Worker’s Compensation
- Personal Injury or Automobile Accidents
- Health Savings Accounts (HSA’s)
- Other Financial Arrangements
Individual or Group Insurance Plans
Our office will file your insurance claims at no charge. Group and private (individual) insurance agreements are between you and your insurance company. It is to be understood and agreed that services rendered and products purchased are charged to you directly and you are personally responsible. We are not certain if your insurance covers chiropractic, although most policies do provide coverage. The amount they pay varies from one policy to another, ranging from 100% to a small percentage. Because of this difference, we require that each patient who wishes to file claims through this office, pay the deductible, the patient’s percentage or co-pay. If there is an overpayment, it will be refunded to you when all insurance payments have been received.
Managed Care Plans (PPO’s & HMO’s)
Most managed care plans require the patient to pay a co-payment or a percentage at each office visit. Dr. Heller participates in most major PPO programs including the following:
- Blue Cross/Blue Shield
- Midlands Choice
- Coventry
- Ohara Managed Care
If you do not see your plan listed, please call to verify coverage since we are added to other plans frequently. Even if we are not a provider for your plan, most plans offer good out-of-network coverage.
Medicare (Part B)
Dr. Heller is a Participating Provider that also accepts assignment from Medicare. Medicare will pay 80% of the allowable fee for chiropractic spinal manipulations that fall within Medicare policy guidelines after your Medicare deductible has been met for the year. The patient is responsible for paying for any services not covered by Medicare, the Medicare deductible (after the Medicare claim has been processed), and the 20% co-insurance. If you have a Medicare supplemental policy, we will file that claim for you also. Supplemental plans usually pay for all the 20% co-insurance, and some may even pay for your Medicare deductible.
Medicaid (Nebraska Only)
We welcome and accept Nebraska Medicaid patients. You will be asked to present your current Medicaid Assistance card to the front desk each month. Our office will submit the forms necessary for reimbursement of your chiropractic care. You may be required to pay a daily co-payment or monthly “share of cost” amount at the time of each visit, unless you are exempt.
Worker’s Compensation
Medical treatment for injuries occurring while on the job are covered 100% by Workers Compensation.
Under Nebraska workers’ compensation laws, you have certain rights in regards to choosing a doctor or changing doctors for treatment of a work related injury. (NWCC Rule 50) Please be sure to discuss your choice of doctor with your employer. Our office will coordinate with your employer and the workers’ compensation carrier for payment of your claims.
Personal Injury or Automobile Accidents
Chiropractic treatment of injuries from an automobile or other accident is covered by liability insurance. Coverage for automobile injuries is paid by the auto insurance of the other party or by the medical coverage portion of your own auto policy. Additional coverage may also be available through your health insurance coverage.
Health Savings Accounts (HSA’s)
All out of pocket expenses for chiropractic services are qualified expenses through Flexible Spending Plans, Cafeteria Plans or HSA’s. Funds from these accounts may be used for co-pays, co-insurance (patient’s percentage), deductibles or other chiropractic expenses not covered by insurance.
Other Financial Arrangements
If you don’t not have any insurance or a high deductible, there are still several options for payment of your care.
Payment types accepted include:
- Cash, Checks, Debit Cards & Credit Cards (Visa, Master Card & Discover)
- Payment plans set up for your convenience
- PACE Card – A qualifying time of service discount when you pay in full at each visit
- Financial Hardship – An Individual Consideration Contract based on income and family size (Must meet income guidelines, proof of income required)
